I second meer.net.  Their key differentiator is that you always get a
clueful person on the phone.  We have been using them for about 9 months
and we are really happy.  In that time we have had 2 problems with our
DSL service - neither one of them caused by them.  One was with the modem
- they reproduced an intermittent problem in their lab and had a
workaround to us pretty quick.  The other was actually a problem with
the circuit - again not their fault - but they were able to point in the
right direction to look pretty quickly.
